Timing and signed reasons
The form asks for each pupil's supervision commencement date and the accounting entities involved, together with reasons supporting consent. The public accountant signs the accuracy declaration. The stated ordinary application window is within 30 days after the hot review order takes effect. A separate February 2015 transitional deadline concerns older approved-principal arrangements and is not an additional current window.
