Evidence of the owner, occupier and use
Provide both the owner’s and occupier’s identifiers, names and charity-registration status. The form requests constitutions or equivalent documents, approved planning use and specific uses of the premises. Separate floor areas are requested for activities such as childcare, columbarium, shops, staff quarters and clubhouses; a floor plan can support the breakdown. It also asks whether rent is collected from the occupier.
Rental and parking details
Disclose all let-out spaces, including arrangements such as ATM or base-station sites, contracted future rentals, variable rent and service charges. Chargeable parking requires lot counts, receipts and expenses. The owner or an authorised person signs the declaration. Completing the form is an exemption application, not proof that every charitable owner or every part of the property is automatically exempt.
