Who completes the application
Form F04-03-SRS concerns penalty-free premature withdrawal from a Supplementary Retirement Scheme account upon bankruptcy. It is completed by the SRS operator when the Official Assignee approaches the operator about funds in the member’s account. The estimated completion time is three to five minutes once the member particulars and Notice of Bankruptcy order are ready. This is an operator-submitted process rather than a general member withdrawal form.
Member information and evidence
Record the member’s full name and NRIC, FIN or passport number as registered with the SRS operator, deleting identifier descriptions that do not apply. Supply the address, Bankruptcy Order number and bankruptcy date. A copy of the Notice of Bankruptcy order issued by the Official Assignee or trustee in bankruptcy must accompany the application; the attachment requirement is repeated in the footnote.
Operator certification and IRAS decision
The authorised officer supplies a name, designation, contact number, signature and date; the form also asks for the SRS operator’s name, address and official stamp. Leave the office-use section to IRAS, which records approval or refusal, reasons and the approving officer’s certification. The form asks for a penalty-free withdrawal decision; it does not itself promise approval or state that the withdrawal has no income-tax consequences.
