Amount, authority and company particulars
Enter company name, UEN and address. The undertaking is S$5,000 per holder or an amount specified by the Controller, for issuing passes to persons listed in the Letter of Guarantee.
The form cites the Employment of Foreign Manpower Act (Chapter91A) and Work Passes Regulations regulation12. It is signed once, with the obligation continuing while the guarantee is valid.
Seven undertakings
The seven undertakings extend beyond purchasing a bond.

Changes and forfeiture
The Controller may amend, remove or introduce conditions and publish effective dates electronically or online. The company must keep informed.
A breach can cause partial or full forfeiture for that person. Partial forfeiture does not extinguish the right to forfeit the remainder for the same or another breach.
Signature, witness and seal
A proprietor/partner signs for sole proprietorships/partnerships and an ACRA-registered director for private companies. Employee delegation requires written MOM authorisation. Complete signer name/designation/signature, company/date and witness name/address/signature.
A common seal is optional under Companies Act41A. Without it a director signs with a witness under41B(1)(c). The regulatory-condition definition refers to Work Passes Regulations2012.
