Identification and truthfulness
Complete the form for issuance and enter the application reference from MOM’s approval/acknowledgement email. The applicant declares that application information is true to their knowledge and supporting copies are genuine copies of originals. The health statement says the applicant has not had/is not suffering from AIDS, HIV infection or TB. Read and answer the actual declaration truthfully.
Stay, maintenance, repatriation and conduct
The applicant accepts responsibility for their stay, maintenance and repatriation, indemnifies Singapore for repatriation costs for self/dependants, and undertakes compliance with imposed quarantine/medical surveillance under the specified Immigration Regulations provisions. The form also undertakes not to misuse controlled drugs or engage in political/other activities that would make the person undesirable or prohibited under immigration law.
Information verification and disclosure
Consent permits the government/statutory authorities to obtain and verify information, including medical records, from persons, organisations and sources; display it in MOM work-pass systems; and release it to authorities, agents and relevant organisations/persons for assessment and pass administration. It includes displaying pass details when the MOM app scans the card.
Income and contact-data consent
The Comptroller of Income Tax may verify declared current/renewal income from the current YA assessment. If unavailable or unfinished, the two previous YAs may be used; results go to the Controller of Work Passes. Contact details may support emergencies and employment/pass messages and be shared with government agencies/statutory authorities for that purpose. Personal details may be shared with the Singpass issuer for an eligible future account.
Singapore Global Network consent
The form contains consent for sharing application information with EDB’s Singapore Global Network and confirms reading its privacy statement and joining the network. Members can unsubscribe from newsletters at any time or withdraw by writing to [email protected]. This is part of the actual declaration wording; readers should review it before signing.
Continuing duties and signature
Report professional activities at each calendar-year end in MOM’s prescribed format and deadline; notify contact/residential changes within 14 days; obtain approval before starting a restricted occupation. Known false, untrue or materially misleading/omitted information may lead to prosecution, IPA cancellation and ONE Pass revocation. Finish with applicant name, signature and date in DD/MM/YYYY format.
