Passport and translation
Submit the candidate’s passport personal-particulars page. For a child sharing a parent’s passport, also submit the parent’s particulars page. MOM may request additional documents. Combine every non-English original and its English translation into one uploaded file; a translation provider may assist.

Child vaccination verification
Foreign-born children aged 12 or below need CDA Verification of Vaccination Requirements (for entry to Singapore) for a new DP, including where the previous DP expired or was cancelled more than 30 days ago. Existing DP, LTVP and Student’s Pass holders do not need this document. The requirement applies to biological and legally adopted children.
