Worker and employer declarations
In Work Permit application Part I(A)(b), strike out have/have never as appropriate for criminal-history declaration. In Part II(h), strike out has/has not for agency engagement, completing agency licence/part where applicable. Do not omit the authorised representative’s name.
Bond and medical form
The guide specifies the bond version dated 29 April 2022; complete date and witness name/address/signature. Enter occupation on the medical form and have the doctor strike out the inapplicable Fit/Unfit.
Its doctor-certification example is Part IV of an older form. The 2026 A11 form uses Part V, so follow the actual current form rather than the old numbering.
BCA identity verification and endorsement
Book identity verification at otms.bca.gov.sg using Corppass and choose a slot. Upload BCA-endorsed IPA after verification. From 13 March 2023, a BCA kiosk-printed endorsement letter is also accepted.
ICA e-Pass
For the Frequent Traveller endorsement case, provide ICA’s Electronic Visit Pass. Retrieve it in the official e-Pass Enquiry Portal using the DE number on SG Arrival Card. The guide is dated 28 August 2023 and should be read with current issuance instructions.
