Deadline, overseas loss and theft
Apply within one week of loss, theft or damage. For theft, make a police report. If lost overseas, notify MOM immediately for a letter supporting entry into Singapore. The holder, company representative or appointed EA applies. Most cases are immediate, with collection four working days after approval. MOM may call an interview before replacement or for repeated losses.
GST-inclusive, non-refundable fees
Damage costs S$65.40. Loss or theft costs S$109 for the first occurrence and S$327 subsequently. All are GST-inclusive and non-refundable once submitted. The free digital pass in SGWorkPass is available immediately after setup and reduces reliance on a physical card. If a lost card is found, promptly post it to Ministry of Manpower, 18 Havelock Road, Singapore 059764.
Online representative or counter application
The company representative or EA can use EP eService and pay through GIRO, Visa, Mastercard or Amex; approval letters go to the representative and holder. Alternatively book EPSC, bring original passport and any damaged card, and pay with Visa, Mastercard, Amex, NETS or CashCard. Enable overseas transactions on foreign-issued cards. The counter issues the replacement letter.
Collecting lost or stolen replacement
Collection is at EPSC four working days after approval, without a collection appointment. The holder must attend personally for loss or theft, bringing original passport, replacement letter and any police report; overseas police reports need English translation. Bring the damaged card where applicable.
Authorised collection for a damaged card
Damaged-card replacements may be collected by a representative. Bring authorisation from the holder, the representative’s NRIC/pass card/passport, the damaged card and replacement letter, plus the holder’s original passport.
