Deadline and special situations
The employer or appointed EA applies within one week of loss, damage or theft. File a police report for theft and inform MOM immediately of overseas loss for an entry letter. Repeated losses require an interview appointment with the employer and worker; replacement is subject to approval.
Encourage free, immediately available SGWorkPass after setup. Post a recovered card immediately to MOM, 18 Havelock Road, Singapore 059764.
Fees and application
GST-inclusive, non-refundable fees are S$65.40 for damage, S$109 for first loss/theft and S$327 subsequently. Most processing is immediate; collect four working days after approval.
Apply through WP Online and pay by GIRO or eNETS credit/debit. Print the approved replacement letter and follow its collection instructions.
Collection and representation
Collect without an appointment at MOMSC Hall C with original passport, replacement letter, damaged card if applicable, declaration for loss/theft and police report for theft with English translation if overseas. Lost/stolen cards require personal collection.
Damage permits a representative with company authorisation, their own NRIC/pass card/passport, damaged card, worker’s original passport and replacement letter.
