| Parents | Identity document/FIN, citizenship, birth, race/dialect, occupation/qualification and contacts/address; mother’s live-born count |
| Marriage/informant | Marriage certificate/date/place or unmarried selection; informant relationship and non-parent identity/contact |
| Declaration | Both named parents know and consent; no amendment after submission; name alteration before one year old is a separate paid process |
