Purpose and scope
Form 6E records the applicant, spouse and travel purpose for a Certificate of Identity. It provides an overseas submission and local collection-authorisation route.

Completion and follow-up
Submit personally to ICA or through the relevant overseas mission. The declaration addresses possession of another government’s travel document and verification consent.
