Purpose and completion
This form records biodata, foreign residence/citizenship, education, the last ten years’ work history and family information. Use it when requested for the relevant ICA case, not as a replacement general citizenship application.
Fields and checks
Prepare the following information as applicable to this form.

Declarations and supporting records
Indicate NIL where directed and use additional family sheets if needed. The signed/date declaration and documentary evidence must reflect the actual person and case.
