Who may use the form
The ICA FormSG instructions restrict this form to reporting for someone unable to report personally online or at ICA Services Centre because of medical reasons. For a child without Singpass, the instructions instead direct the parent to ICA Services Centre for reporting/replacement.
Access and supporting records
The public landing page requires Singpass before the form fields can be accessed. ICA’s linked FAQ identifies the supporting records: a doctor’s memo describing the medical condition, the completed lost-NRIC replacement form and relevant LPA or court/deputy authority for mental incapacity. ICA reviews the request and then contacts the submitter.
