Identity and narrow authority
Complete Section A with taxpayer name and NRIC/FIN/ASGD/ITR tax reference, then collector name and identity number/type, which may include passport. This authorises collection at IRAS’s Taxpayer & Business Service Centre, not enquiry or management of tax matters; the form refers to a separate authorisation for those tasks. Complete both sections beforehand because the collector cannot amend the form at collection.
Specify documents and periods
Tick assessments by YA range; Form B1/B/M by YA; Form P with partnership ROB/UEN/ASGD and YA; account statements by date range; or property-tax bills/statements with tax references/official addresses and years. The example YA 2025 covers calendar 2024, so distinguish an income-tax YA from statement dates and property-tax years.
Declaration and documents at collection
Declare information/attachments true, correct and complete, attach a passport/official identity copy bearing particulars and signature, and sign/date with name/contact. The collector presents the signed form, taxpayer original identity document or, if unavailable, a scanned signed passport, and their own original identity. Missing items may lead to rejection. The form estimates five minutes to complete.
Fees and payment
Duplicate assessments/documents attract search and document fees. The form directs readers to the current income/property-tax fee tables rather than giving a numeric amount. Its stated collection payment methods are NETS/Cash Card only; do not turn the blank form into an invented fixed fee or unrestricted payment instruction.
