Upload the personal-particulars page of the candidate’s passport and the company’s latest ACRA business profile or instant information. If the passport name differs from names in other documents, include an explanation letter and supporting evidence, such as a deed poll. Follow MOM’s upload guidelines so document issues do not delay assessment.

Healthcare professionals, lawyers, football players and coaches require supporting documents from the relevant professional body. The full source list is set out below; use the indicated body for the relevant occupation.

Employees in food establishments
Submit a copy of the online page opened by scanning the QR code on the Singapore Food Agency foodshop licence; it must show the licence validity period. A new establishment that has not yet received the licence may submit SFA’s Application for Foodshop Licence letter instead.
Translations and further requests
For a non-English document, upload its original and English translation together as one file. A translation service provider may prepare the translation. MOM may request additional documents during review, so the initial document list is not a guarantee that no further evidence will be needed.
