Who should use this form
The 6 July 2026 form, reference MOM (WPD) DPLTVP(SPE)/20260706, is specifically for dependants of Employment Pass (Sponsorship) candidates or holders. It combines Dependant’s Pass (DP) and Long-Term Visit Pass (LTVP) applications. Dependants of ordinary Employment Pass or S Pass applicants must instead use go.gov.sg/ep-eservice. Open and complete the form in Adobe Acrobat Reader DC because other readers may not display it correctly. MOM estimates about 20 minutes to complete it.
How the applicant and local sponsor divide the work
The foreign applicant and work pass candidate or holder complete Parts 1–6, on pages 2–6, and send the electronic form to the local sponsor. The local sponsor checks these details and asks for corrections where necessary, then completes Parts 7–8 on pages 7–10. Enter “Not applicable” or “N.A” where a field does not apply; do not leave fields empty.
The sponsor uploads the completed form and supporting documents and pays the application fee through the online submission form. The organisation needs a Corppass account. MOM indicates processing within six weeks, although some applications take longer, and allows status checking at go.gov.sg/check-work-pass-application after one week.
Part 1: application and relationship information
Provide the notification email, the work pass candidate or holder’s FIN where applicable, passport name without titles, and any alias that appears in the travel document. Declare previous Singapore Employment Pass, S Pass, Work Permit, Student’s Pass, DP or LTVP applications or stays, entering the relevant FIN or Work Permit/S Pass number. State the family relationship and requested duration: at least one month and no later than the supporting work pass expiry. Approval may be for a shorter period than requested.
Part 2: personal and travel document details
Record the dependant’s passport name and alias, sex, marital status, date of birth, nationality or citizenship and its state/province, country and state/province of birth, and country and state/province of origin. The form defines origin as the place where the first citizenship was obtained by birth or parentage. It also asks for race, religion, travel document type, number, issue and expiry dates, and email address.
Part 3: vaccination verification for children
Part 3 applies to a foreign-born child aged 12 or below applying for a new DP or LTVP. Obtain the Communicable Diseases Agency (CDA) Verification of Vaccination Requirements (for entry to Singapore) document before submission. Declare whether the child currently holds an ICA Student’s Pass and enter the CDA NIR reference number. The document checklist also covers a new application where a previous DP or LTVP expired or was cancelled more than 30 days earlier. All three conditions—birth outside Singapore, age 12 or below, and a new application—must be met for the checklist requirement.
Part 4: immigration, legal and residence history
Answer all ten history questions: refusal of entry or deportation; criminal conviction; prohibition on entering Singapore; Singapore entry with a different country’s passport; entry under another name; former Singapore citizenship or permanent residence; another country’s work visa; study in Singapore; work in Singapore; and long-term Singapore residence other than tourism. For an overseas work visa, provide the issuing country and duration in years and months. Give details whenever any answer is yes.
Part 5: the dependant’s declaration and consent
The dependant signs the declaration; a parent signs for an applicant under 16. It confirms accurate application details and genuine copies, and contains declarations about AIDS, HIV and tuberculosis. The applicant undertakes not to misuse controlled drugs or undertake political or other activities that would make them an undesirable or prohibited immigrant.
The consents allow government and statutory authorities to obtain and verify information, including medical records, from third parties; disclose it to relevant authorities, agents and organisations; display information on MOM systems and pass details when a card is scanned; use and share contact details for emergencies and pass/employment communications; and share details with the Singpass issuing agency if later eligible. The form requires changes to contact details, including residential address, to be notified through MOM within 14 days. Material falsehoods or misleading omissions may lead to prosecution, cancellation of in-principle approval or pass revocation.
Part 6: the supporting work pass holder’s undertaking
The work pass candidate or holder undertakes responsibility for the dependant’s upkeep and maintenance in Singapore and for ensuring compliance with quarantine and medical surveillance imposed under Immigration Regulations 8(2A) and 8(4). They confirm the truth of the application and supporting copies, acknowledge potential prosecution for material falsehoods or omissions, and sign with their FIN and date.
Part 7: local sponsor responsibilities
The sponsor must be the same company that sponsors the supporting work pass applicant or holder. Its authorised human resources employee or a representative at managerial level or above signs with designation, NRIC/FIN, phone number, company address and date.
The sponsor confirms the stated purpose and accuracy to its knowledge, indemnifies the Government for repatriation charges or expenses, and retains relationship certificates throughout the work pass holder’s employment. MOM may request these records and revoke the pass if the records conflict with the declaration or cannot be produced. The sponsor also agrees to card-scan display and declares that, if approved, it will apply to MOM for the dependant’s entry subject to prevailing entry requirements.
Part 8: employment agency declaration
Complete Part 8 only when an employment agency was used. Its personnel declare that they explained the application and declaration to the dependant, supporting work pass candidate or holder, and local sponsor. They certify the application, declarations, appeals and copies as true to their knowledge, and provide agency name, licence number, UEN, personnel name and number, signature and date.
Upload formats, limits and payment
Submit at go.gov.sg/submit-dependants-of-ep-sponsorship. Payment options are PayNow, Visa, Mastercard and Amex. Non-English documents must include an English translation, which may be prepared by a translation service provider. MOM may request additional documents during review; false salary or qualification declarations and forged work pass documents are offences under the Employment of Foreign Manpower Act.

Supporting documents for a Long-Term Visit Pass
For a common-law spouse, any one of the three listed forms of recognised-relationship evidence can be used. A home-country affidavit must be notarised there. For an unmarried stepchild under 21, both custody and the other biological parent’s no-objection letter are required; the child vaccination conditions also apply.
