Supporting forms
The directory lists a full medical examination form for a Singapore-registered doctor to complete for issuance; a GIRO application for pass administration fees; a separate GIRO application for levy payments; and a prior-approval application for hiring Non-Traditional Source workers under conservancy schemes. The two GIRO forms have different purposes.
