The employer must maintain insurance
Buy and maintain medical insurance for every S Pass employee before issuing or renewing the pass. Do not charge the insurance premium to the worker. Cover inpatient treatment and day surgery, including illnesses or conditions unrelated to work, with an annual claim limit of at least S$60,000. Any separate sub-limit for inpatient care, day surgery or each condition/disability must also meet that minimum annual limit.
Enhanced cover from July 2023
For policies, renewals or extensions starting on or after 1 July 2023, the enhanced minimum annual claim limit is S$60,000. For the portion of a claim above S$15,000, insurers bear 75% and employers 25%. This insurance-to-employer allocation is separate from the conditions governing any arrangement for a worker to contribute to medical bills.
Additional requirements from July 2025
Policies, renewals or extensions starting on or after 1 July 2025 must also use the standardised allowable exclusion clauses, distinguish premiums between people aged 50 or below and those above 50, and provide direct insurer payment to hospitals once the claim is admissible. Engage insurers early to compare products that meet all applicable requirements.
When a worker may contribute to medical bills
A medical-bill co-payment arrangement is permitted only if every condition is met: the amount is reasonable and no more than 10% of the worker’s fixed monthly salary; the payment period is no longer than six months for every two years of employment; and the employment contract or collective agreement expressly provides the option with the worker’s full consent. These conditions do not permit passing the insurance premium to the employee.
