Miscellaneous Work Pass Application Form: Complete Field and Declaration Guide

A guide to all nine parts of MOM’s ten-page application form, including candidate history, activities, sponsor undertakings, signatures and upload limits.

Form version and three-stage submission

This guide follows the ten-page form bearing reference MOM (WPD) MWP/20261002. Open and complete the PDF with Adobe Acrobat Reader DC; the source warns that other readers may not display it correctly. Allow approximately 30 minutes to fill it in. Complete the electronic form, arrange all required signatures, then upload it with supporting documents and pay through the online form. The organisation needs a Corppass account for online submission.

Consent, completeness and processing time

Obtain the candidate’s written consent before applying. Do not leave fields blank; enter “Not applicable” or “N.A” where appropriate. Most applications take around six weeks, with some requiring longer. Status can be checked at go.gov.sg/check-work-pass-application. Event organisers should wait for approval before publicising the candidate’s participation.

Part1: Candidate identity and previous Singapore identifiers

Enter the FIN or Work Permit/S Pass number if the candidate previously applied for or worked on an Employment Pass, S Pass or Work Permit, studied on a Student’s Pass, or stayed on a Dependant’s Pass or Long-Term Visit Pass. Give the name exactly as on the travel document without titles such as Mr, Professor or Doctor; enter an alias only if shown on that document. The form also asks for birth date, sex, marital status, nationality/citizenship and province, birth country/region and province, origin country/region and province, the last address there, race and religion. Origin means where first citizenship was acquired by birth or parentage.

Part1: Travel document, contact and Singapore address

Record travel-document type and number, issue date and expiry date; supply the candidate’s email address and telephone number. If the candidate is currently staying in Singapore, provide the residential block/house number, street, unit, building name and postal code. The form uses DD/MM/YYYY for dates.

Part2: Education and membership history

Start with the highest qualification and reproduce details as shown on the educational certificate. Two qualification entries provide space for the awarding institution, country/region and province, qualification including honours class/division where relevant, specialisation, faculty, study dates and study mode. Indian qualifications also require whether study was at the main campus or an affiliating college. Record society or organisation memberships over the past five years, including the organisation, position and dates; the form provides two membership entries.

Part3: Employment history

State total work experience and total experience relevant to the occupation to be entered in Part4C, in years and months. List employment starting with the most recent period, recording start/end dates, company, country/region and province, position and duties. Periods of unemployment must also be stated.

Part4A: Activities, exact dates and the six-row limit

List each activity chronologically with its name and description, address, and exact start/end dates. The combined activity duration cannot exceed 60 days. The form has six rows: if there are more than six activities, provide the additional details in a separate document and upload it with the form. If further activities are required after submission, another application must be submitted.

Part4B–4C: Fixed monthly salary and occupation

Enter salary amounts specified in the employment contract. Fixed monthly salary is basic monthly salary plus fixed monthly allowances, excluding payments that vary month to month. The form illustrates S$4,500 basic salary plus S$500 fixed allowances equalling S$5,000 fixed monthly salary. This is an illustration, not a stated qualifying salary threshold. Enter the occupation and consult the linked standard-occupations list if needed.

Part5: Sponsoring organisation and contact people

Provide the sponsor company, society or organisation’s registered name, correspondence address, UEN and telephone number. A contact person’s name, email and phone are mandatory so MOM can contact the applicant. Identify the sponsor signatory and designation in Part5C; these details populate the local-sponsor declaration.

Part6: Other declarations and disclosure

Answer whether the candidate has been refused entry or deported anywhere, convicted by a court anywhere, barred from Singapore, entered Singapore on another country’s passport or under another name, been a Singapore citizen or permanent resident, worked or studied in Singapore, or stayed here long term rather than as a tourist. Give details for every “Yes” answer.

Part7: Candidate’s declaration and signature

The candidate confirms that Parts1–3 and6 were supplied by them and are true and correct. Materially false information or misleading material omissions may lead to prosecution. The candidate undertakes not to undertake activities inconsistent with the purpose of the pass, or employment, business or an occupation without written consent from the Controller of Work Passes. The declaration also authorises the Singapore Government to obtain and verify information from other sources when assessing the application and residence. The candidate signs and dates it; the name is generated from Part1A.

Part8: Local sponsor’s declaration and responsibilities

The local sponsor confirms the purpose, accuracy of Parts4–5, written candidate consent and, to its knowledge, the candidate’s statements. Consent must be produced when requested. The sponsor undertakes responsibility for the candidate’s stay, maintenance and repatriation; indemnifies the Government for repatriation costs incurred for the candidate or dependants; and takes responsibility for compliance with quarantine and medical surveillance imposed under Immigration Regulations regulation8(2A). The signatory must be authorised HR personnel or someone holding at least a managerial role in the sponsoring company, and enters their signature, date and NRIC/FIN. Name and designation are generated from Part5C.

Part9: Employment agency declaration, where applicable

Complete this part only if an employment agency was used. Agency personnel declare that the application and declaration contents were explained to the candidate and local sponsor, that information in the application, declarations and any appeals is true to their knowledge, and that supporting documents are true copies of originals. Provide the agency name, licence number, UEN, personnel name and number, signature and date.

Upload checklist, formats and file-size limits

Use go.gov.sg/submit-mwp to upload the completed form and supporting documents and pay by PayNow, Visa, Mastercard or Amex. The three upload categories and limits are shown below. Include travel-document pages confirming amendments to personal particulars, such as a changed name or expiry date. Non-English documents must be accompanied by an English translation, which may be prepared by a translation service provider.
