Reporting and approval
The employer or appointed agent applies within one week. Theft requires a police report, and overseas loss immediate notice to MOM for an entry letter. Approval may involve an interview. If found later, post the old card immediately to Ministry of Manpower, 18 Havelock Road, Singapore 059764. A free SGWorkPass digital pass helps reduce physical-card risks.
Charges and application
Damage costs S$65.40, first loss/theft S$109 and subsequent loss/theft S$327, including GST and non-refundable. Apply in WP Online and pay by eNETS credit or debit. Three or more losses require the separate replacement request, not WP Online. Most cases process immediately; print the replacement letter on approval.
Collection requirements
Collect at Hall C four working days after approval without appointment. Bring the helper’s original passport, replacement letter, applicable damaged card, completed declaration for loss/theft and theft police report; translate overseas reports into English. Despite the general statement that employer or helper may collect, the specific rule requires personal collection for lost/stolen cards. Damage can be collected by an authorised person with employer authorisation, their NRIC/card/passport, the damaged card, helper’s passport and replacement letter.
