Deadlines and reporting
The employer or agent must apply within one week. Report theft to police. For overseas loss, inform MOM immediately for an entry letter. MOM may request an interview and approval is required. Return a subsequently found card by post immediately to Ministry of Manpower, 18 Havelock Road, Singapore 059764. Use the free digital pass in SGWorkPass where appropriate to reduce physical-card risks.
Fees and submission
Damage costs S$65.40; first loss/theft S$109; subsequent loss/theft S$327, including GST and non-refundable after submission. Apply in WP Online and pay by GIRO or eNETS credit/debit. For three or more losses use the separate MOM request rather than WP Online. Most cases process immediately; print the approved replacement letter for collection instructions.
Collection after four working days
Collect at Hall C four working days after approval, without appointment. Bring original passport, replacement letter, damaged card where relevant, a completed declaration for loss/theft and the police report for theft; overseas reports require English translation. Loss/theft replacements require personal collection. Damage may be collected by an authorised person with company authorisation, their NRIC/card/passport, damaged card, worker’s original passport and replacement letter.
