Deadline and special situations
Apply within one week of loss, theft or damage, and file a police report for theft. Inform MOM immediately after an overseas loss so it can issue an entry letter.
The employer or appointed EA applies; EntrePass or PEP dependants may apply over the counter at EPSC. MOM may require an interview before issuance or for repeated losses. Post a recovered lost card immediately to MOM, 18 Havelock Road, Singapore 059764.
The digital pass in SGWorkPass is free and immediately available after setup, reducing reliance on a physical card.
Fees and processing
Fees include GST and are non-refundable. Most requests are approved immediately; collection is four working days after approval.

Application routes
Provide the details through EP eService and pay by GIRO, Visa, Mastercard or Amex. No refund is allowed after submission. On approval, both parties receive a card replacement letter by email with collection instructions.
An S Pass dependant who has lost the card three or more times must submit a replacement request to MOM instead of using EP eService.
Collection locations and documents
Collect four working days after approval without an appointment. EP, EntrePass and PEP dependants use EPSC; S Pass dependants use MOMSC Hall C.
Bring the original passport, replacement letter, damaged card where applicable and police report for theft. Translate overseas police reports into English. Lost or stolen cards must be collected personally.
For damaged cards, a representative may collect with the company’s authorisation letter, their own NRIC/pass card/passport, the damaged card, holder’s original passport and replacement letter.
