Deadline, overseas loss and fees
The holder applies within one week of loss, theft or damage. Theft also requires a police report. Report overseas loss immediately so MOM can issue an entry letter. Damaged-card replacement costs S$65.40; first loss/theft S$109; later loss/theft S$327, including GST and non-refundable. Most requests are immediately processed; collection is four working days after approval. MOM may interview repeat-loss holders. A recovered lost card must be posted to MOM, 18 Havelock Road, Singapore 059764.
EPSC application appointment
Book EPSC replacement and bring the holder’s original passport plus damaged card or theft police report as applicable. Overseas police reports require English translation. Pay by Visa, Mastercard, Amex, NETS or CashCard; activate foreign-issued cards for overseas use. Receive the replacement letter. SGWorkPass’s free digital pass becomes available on setup and reduces physical-card risk.
Collecting a lost or stolen card
No collection appointment is needed after four working days. The holder must attend personally with original passport, replacement letter, completed lost/stolen-card declaration and theft police report if applicable.
Collecting a damaged-card replacement
The holder may attend with original passport, replacement letter and damaged card. Alternatively, an authorised person brings the holder’s authorisation, their own NRIC/pass card/passport, replacement letter and damaged card. The LTVP page additionally requires the holder’s original passport for authorised collection; this item is not in the DP authorised-collection table.
