What the release reports
Muhammed Noredzuan Bin Othman received four months for negligence causing a worker’s death during January 2016 pipe repairs.
Details and conditions
He endorsed a test checklist without the required line walk. A wrongly positioned valve trapped pressurised gas, ejecting a clamp connector into the worker.
Context and follow-up
Generco Engineering had separately been fined S$180,000. The case shows that signing a checklist cannot replace physically checking test boundaries and valve positions before pressure work.
