What the release reports
Two workers died after a May 2024 tank-cleaning incident involving hydrogen sulphide exposure; another was treated and discharged. Sensors had detected unsafe gas levels and sounded an alarm.
Details and conditions
On 16 July 2026, Stargroup faced one risk-management charge and its confined-space permit manager Lim Beng Hock faced twelve due-diligence charges. A PUB officer received a stern warning, while PUB was offered composition for earlier inadequately ventilated entries. These actions are distinct from criminal convictions.
Context and follow-up
Confined-space work requires prior risk assessment, atmospheric testing, appropriate respiratory protection and a trained standby person outside. The earlier stop-work order was lifted in August 2024 after conditions were met; this release concerns subsequent enforcement.
