Passport and translation requirements
Provide the applicant’s passport particulars page. A child sharing a parent’s passport also requires that parent’s particulars page. MOM may request further evidence during review.
Upload each non-English original and its English translation together as one file. A translation service provider may prepare the translation. Follow the linked upload requirements to avoid document-related delays.
Relationship-specific evidence
The required relationship evidence differs by category; the sponsor’s work pass alone does not establish the family relationship.

Children requiring vaccination verification
A foreign-born child aged 12 or below applying for a new DP needs the CDA-issued Verification of Vaccination Requirements for entry to Singapore. This applies to both birth and legally adopted children in those circumstances.
A new application includes a previous DP that expired or was cancelled more than 30 days earlier. Existing DP, LTVP and Student’s Pass holders do not need this document. Check age, birthplace and application status together.
