Passport and translations
Provide the applicant’s passport particulars page and follow MOM’s upload guidelines. Further documents may be requested during review.
Upload a non-English original and its English translation together as one file. A translation service provider may prepare the translation.
Common-law spouse: one of three alternatives
Provide any one of three alternatives: a common-law marriage certificate copy; an affidavit by the holder and spouse declaring that their relationship is recognised under their home country or region’s laws, notarised there; or a letter from either partner’s home-country or region embassy acknowledging that legally recognised relationship.
Evidence for other relatives
Prepare both custody evidence and the other biological parent’s consent for a stepchild.

Childhood vaccination verification
A foreign-born child aged 12 or below applying for a new LTVP needs CDA’s Verification of Vaccination Requirements document for entry to Singapore.
A new application includes one where the previous LTVP expired or was cancelled more than 30 days earlier. Existing DP, LTVP and Student’s Pass holders do not need this document.
