Time limits and requests
After 30 days, use an online amendment request for editable fields. Medical leave can be amended directly within one year of submission, then requires an online request. The fields below cannot be changed by the informant at any time after submission.

Duplicate deletion and claim withdrawal
Use the deletion-request form, select Deletion/Removal of Duplicate Submission and supply both new and existing report references. Do not file a replacement unless MOM asks. To withdraw a compensation claim, contact the insurer directly rather than treating report deletion as withdrawal.
