What the release reports
M.C. Packaging faced two WSH charges over 2016 fatal incidents. Its former factory manager had also been charged and pleaded guilty at the announcement.
Details and conditions
One worker was trapped cleaning a palletiser; another was struck by a reversing forklift. Alleged failures included energy isolation, guarding, supervision and traffic-risk controls.
Context and follow-up
Families each received around S$200,000 insurance compensation. Lock-out/tag-out prevents unintended machine activation, while traffic plans separate vehicle and pedestrian risks. Compensation does not eliminate safety accountability.
