Replacement documents
For a lost or damaged citizenship certificate, prepare a written replacement request and a statutory declaration, IC, original birth certificate with a copy, and a colour photograph taken within three months. Non-English documents need an accepted translation.
In-person procedure
Submit at ICA Services Centre, 2 Crawford Street. A minor must be accompanied by a parent or guardian both for the application and collection. The statutory declaration is available at ICA.
Fee and collection
Replacement costs S$20 and is normally ready within two weeks. This replaces the certificate; it is distinct from a citizenship-status confirmation letter.

Written independently from ICA’s English original. The linked source supplies the complete document and subsequent updates.
