Workright Inspection Annexes: Employee Records and Authorisation

Annex A: company and employee details

Annex A-1 records the business name, UEN, company and representative contacts, designation, signature and submission date. Annex A-2 asks for at least five employees with at least three months of continuous service, including at least two hired on or after 1 April 2016. Consult the assigned inspector if the business has no employees meeting the stated selection criteria.

Annex B: appointing a representative

Use the authorisation letter where a director, partner or sole proprietor delegates representation. Identify the business, authorising person and representative, state the relationship and inspection date, and obtain both signatures. The form recognises electronic signatures made with the intention of signing.

Annex C: the document checklist

For each selected employee, mark availability and page counts for the contract/KETs, itemised pay slips and identity/work-pass copies, then add the employee-details form and authorisation letter where needed. Align the stated salary period with the payroll evidence. Record missing documents honestly.

Declaration and retaining originals

The signatory confirms the electronic records are originals or true copies, maintained through the business’s regular processes, and usable as evidence. Retain the originals submitted in soft copy for at least one year and produce them when requested. False information can lead to investigation or enforcement; an electronic submission receives an email acknowledgement instead of an inspector’s physical receipt signature.
