What the release reports
Shell Eastern Petroleum was fined S$400,000 after a 2015 refinery fire burned six workers.
Details and conditions
Hot cutting occurred near cold work draining hydrocarbons. Vapour met cutting sparks because the simultaneous jobs were not coordinated or checked for compatibility.
Context and follow-up
Emergency responders extinguished the fire within thirty minutes, but that response did not cure the underlying oversight failure. Major-hazard occupiers must manage interactions between contractors and tasks, not assess each activity in isolation.
