Fatal lifting and blame-shifting: supervisor jailed

MOM publication date:

What the release reports

Tay Tong Chuan received sixteen weeks: eight for unsafe lifting instructions and eight for obstructing justice. ZAP Piling had earlier been fined S$290,000.

Details and conditions

An unplanned crane lift struck unsecured casings and killed a worker. Tay and a manager tried to make another operator accept blame, but a phone recording exposed the attempt and that worker’s charge was withdrawn.

Context and follow-up

Proper lifting plans and truthful investigation records are both essential. Safety responsibility cannot be shifted through coercion; the original release explains how the recording changed the investigation.
